Core Principles and Team Structure
To support successful implementation, the AIMS Center highlights the Evidence Base for Collaborative Care (CoCM) and its Core Principles, such as Measurement-Based Treatment to Target and Systematic Caseload Review. The Behavioral Health Ecosystem Team Structure is composed of the following members:
- Patient
- Behavioral Health Care Manager (BHCM)
- Primary Care Provider (PCP)
- Psychiatric Consultant
