The home care coordinator provides and demonstrate ongoing expertise in Joint Commission, CMA, State, Federal and other regulatory requirements affecting the medical staff organization. Duties and Responsibilities

Sample responsibilities for this position include the following tasks:

  • Perform the initial assessment, annual reassessment and change of condition assessments of participants in their home environment which may include supportive housing in nursing and assisted living facilities and coordinates the plan of care with appropriate resources.
  • Provide treatments and health education for participants and or caregivers in their home.
  • Direct the provision of personal care, nursing rehabilitation tasks and other work of Home Health Aides and participant caregivers in the home.
  • In conjunction with the Director of Nursing, act as the liaison between the contracted home health agency and the center.
  • Prioritize duties on a daily basis.
  • Obtaining information to verify insurance coverage and inform the patients.
  • Identifying primary and/or secondary payer of services.
  • Participating in establishing the plan of treatment through direct coordination of care.
  • Communicate pertinent information and instructions for specific physicians order to health team members, including completion of the interdisciplinary referral.
  • Facilitate the recording of the face to face encounter.

Patient Care Coordination

Role of the Patient Care Coordinator is responsible for devising effective patient care plans, addressing the patients’ concerns, inventory management, and performing various administrative duties. Key responsibility areas include:

  • Collect information about the patients’ medical history at the time of admission.
  • Formulate the patient care plans and get them approved by the doctors.
  • Maintain and update the patient records in the database.
  • Administer the patients’ medical condition and document the vital symptoms.
  • Process the bills and payments and raise the claim request to coordinate with the insurance agencies.
  • Manage the inventory and restock the required clinical supplies.

Qualifications and Educational Requirements

Qualifications for a job description may include education, certification, and experience. Typically a job would require a certain level of education.

  • Education: Employers hiring for the home care coordinator job most commonly would prefer for their future employee to have a relevant degree such as Bachelor's and High School Degree in Nursing, Education, Health, Business, Technical, Graduate, Social Work, School of Nursing, Healthcare, or Healthcare Administration.
  • Certifications: Licensing or Certifications for Home Care Coordinator include: CPR, BLS, COS, OASIS, CNA, CCP, CM, AHA, CRRN, ANCC.
  • Experience: Prior experience of at least 2-3 years in the medical field is required.

Desired Skills and Expertise

Desired skills for home care coordinator include an understanding of OSHA and other federal/state regulatory agencies guiding healthcare. Additionally, the candidate should possess:

  • Sound medical knowledge to develop the patient’s nursing and care plans as per their medical condition.
  • Incredible communication and interpersonal skills to work together with the treating consultants and other medical staff.
  • Remarkable mentoring skills to guide and train the new hires.
  • Excellent organizational skills in order to manage the calendar and maintain the patient database.
  • Strong computer literacy skills including MS Office (Word, PowerPoint, Excel and Outlook) and working knowledge of database management systems.
  • Compassionate nature in order to empathize with the patients and their relatives.
